Case Management for Home Care

Case Managers assists clients with chronic health care needs, and their family and caregivers, to obtain home and community care services.
Case Managers assist clients with chronic health care needs to obtain home and community care services such as Adult Day Services, Home Support, Assisted Living and Residential Care. Case Managers are available to help with:
  • Identifying care needs, options, and costs
  • Developing a care plan that is reviewed and adjusted as care needs change
  • Consult with other healthcare professionals to ensure the clients needs are met
  • Arrange and coordinate appropriate services, and identify how long they are required
  • Determine how the family and community can support the individual with their needs
  • Case Managers can also work with to ensure smooth transitions between hospital and home, assisted living, or long-term care
Access and Cost of service:
  • Primary healthcare provider referrals are accepted (providers see below), OR the client may call their local Community Access Line to learn about what services are available in their area and how to access them. Please see location telephone numbers and hours below.
  • Case management services are provided at no cost. Note: care at home can involve other costs that the client is responsible for including: their own medicines, any dressing supplies, and if applicable renting/purchasing equipment (walkers, bath seats, wheelchairs, or special cushions for wheelchairs)
  • Note: sometimes the Red Cross has these available to borrow for a short period of time. Read the Island Health Accessing Community Health Services webpage to learn more about costs and eligibility
